I'm looking for some help to replace our old compass card. Due to damage I am unable to complete the process.

Does anyone have an original card they could scan (front and back) and e-mail it to me or post it on the forum perhaps for all to use?

I attach my old card so that others may find it easier to identify the item I am seeking.
